    Medication Therapy Management

Providing patient-centered focus of care to reduce medication errors and cost.

These services are distinct from medication dispensing by focusing on a patient-centered process of care. Services encompass the assessment and evaluation of the patient’s complete medication therapy regimen, rather than focusing on an individual medication product. Our pharmacists work to identify unforeseen side-effects or problems with medication regimens to help provide different drug recommendations as well as guidelines for identifying future issues and tips for taking medication.

Testimonial

"Access to patient care services continues to be a major problem for many patients. Barriers to health services include high cost of care, inadequate or no insurance coverage, lack of availability of services, lack of culturally competent care, lack of transportation, and lack of a family and friend support system for self-care. These barriers lead to unmet health needs, delays in receiving appropriate care, inability to get preventative services, financial burdens and preventable hospitalizations. Telehealth is a mechanism whereby you can cost effectively increase your bandwidth to deliver services and increase connectivity with patients to improve access to comprehensive, quality patient care services and improve health outcomes."
