    Completing medication reviews to optimize medication regimens.

    These services are distinct from medication dispensing by focusing on a patient-centered process of care. Services encompass the assessment and evaluation of the patient’s complete medication therapy regimen, rather than focusing on an individual medication product. Our pharmacists work to identify unforeseen side-effects or problems with medication regimens to help provide different drug recommendations as well as guidelines for identifying future issues and tips for taking medication.

In the Chronic Care Management program, patients are paired with a healthcare navigator to help understand, manage, and achieve a healthier lifestyle. Navigators work one-on-one with patients to create care plans and goals related to chronic conditions by working alongside patients, caregivers, and healthcare providers. This program also helps patients manage daily concerns related to chronic conditions.
