Qualify through Medicare Part D
If you currently use Medicare Part D as part of your insurance, you may qualify if you meet any of the below requirements.
- Spend $4,044 in out of pocket expense in annual medication costs through Part D plans
- Take multiple medications covered by your Part D plans
- Have one or more diagnosis of chronic diseases including but not limited to: Alzheimer’s disease, chronic heart failure, diabetes, dyslipidemia, end-stage renal disease (ESRD), hypertension, respiratory diseases (such as asthma, chronic obstructive pulmonary disease (COPD) or chronic lung disorders), bone disease (such as osteoporosis, osteoarthritis or rheumatoid arthritis), mental health conditions (such as depression, schizophrenia, bipolar disorder or chronic and disabling disorders)
Qualify through Insurance
Insurance companies including but not limited to Aetna, Cigna, SilverScript, Blue Cross Blue Shield, and CareSource, may provide these services through your plan. For your plan’s specific criteria, you should review your handbook or inquire through your plans customer service.
