About Us

Telehealth centers exist to fill a gap in health services for many people. Groups such as senior citizens, those with travel limitations and rural populations benefit from the use of telecommunication technology to enhance the delivery of their care. Telehealth centers can reduce the cost and time burden of travel for patients, identify health risks sooner and increase access across the board to healthcare.

Findlay’s Telehealth Center operates through the combined efforts of its full-time staff, local pharmacists and the University of Findlay. They work to serve patients who need extra care and assistance in medication therapy management and chronic care management. University of Findlay students working under the supervision of the staff of licensed pharmacists communicate with patients in a variety of ways. The Center keeps track of patient’s medications and identifies medication interactions or lifestyle choices that may cause further issues with their illness.

 

History

The Telehealth Center was established in 2014 as part of the University of Findlay College of Pharmacy. The center has grown from serving patients local to the state of Ohio to more than 50,000 nationwide. The center, established on Medication Therapy Management services, now offers an extensive menu of options including Medication Therapy Management (MTM), Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transition of Care (TOC), Contact Tracing (CT)/COVID Testing, Long Term Care (LTC)/Remote Data Entry, Mail order Pharmacy, Immunization Clinics, Remote Operational Support, and Medication Optimization

Telehealth Director Sothea Phon earned her certificate in Medication Therapy Management through the American Pharmacist Association and has also achieved her certificate in Chronic Care Management. She began working at the Telehealth Center in May 2017.
