The University of Findlay College of Pharmacy Telehealth Center offers services to aid in a healthy and safe lifestyle for our patients. The combined efforts of the Medication Therapy Management (MTM), Chronic Care Management (CCM), and Transition of Care (TOC) departments provide a complete patient center focus of care from the beginning of medical diagnosis through a complete life style adjustment.
